- You will be contacted by a FAST representative within 10 business days if this is not an emergency.

Work Request

1. For urgent issues call the FAST Coordination Center directly at 687-2500.
2. Select "Submit a Work Request" located in the left column.
3. Make your selection using the drop-down menus on the page, and then click on the arrow button. Please fill out each page in this manner. 
4. Fill out the screens completely. Ensure all fields marked with an * are completed or the request will be unable to be submitted. When completed, click the submit button.
5. Work requests should be as detailed as possible, and should only contain one maintenance request. For example, replace burned out lights and fix leaky toilet requests should be submitted separately.
6. Once you submit the work request, you will receive both an on-line notification of the request and an email acknowledging the request.

Service Request

1. Select "Submit a Service Request" located in the left column. Make your selection using the drop-down menus on the page, and then click on the arrow button. Please fill out each page in this manner. 
2. Fill out the screens completely. Ensure all fields marked with an * are completed or the request will be unable to be submitted. When completed, click the submit button.
3. Once you submit the service request, you will receive both an on-line notification of the request and an email acknowledging the request.
Please note, however, that Facilities & Safety will not be held responsible for any unauthorized use of a PeopleSoft Chartfield charge provided to us by your departmental staff via F.A.S.T Request.

Access Request

1. Select "Submit an Access Request" located in the left column. Make your selection using the drop-down menus on the page, and then click on the arrow button. Please fill out each page in this manner. 
2. Fill out the screens completely. Ensure all fields marked with an * are completed or the request will be unable to be submitted. When completed, click the submit button.
3. Once you submit the access request, you will receive an on-line notification of the request and an email will be sent to both the keyholder and SACC acknowledging the request.
Only Single Access Control Cordinators (SACCs) or their designees are allowed to approve requests through this form. Please refer to our SACC list here.

Search by Number

1. On left side of the screen under Search by Number, choose "Request Number" or "WO Number".
2. Type in the number in the blank field provided. Click the arrow button to search. Your Work Order number (WO Number) starts with FM, FS or LS.
3. You will then see the current status of your request. If the request was accepted, the Work Order number is provided.
4. To view detail and status, simply click on the Work Order number.
